Anyone claiming expenses Galway Hockey Club now needs to complete this form in order for their claim to be processed - process is outlined at the top of it.

Please complete all fields in 'Submitters Details' & 'Expense Details' sections.   Return your signed expense claim with accompanying receipts (stapled to form) to Eugene O'Donnell:     by post to 14 Carragh Drive, Salthill, Galway     by hand at the pitch     by email to This e-mail address is being protected from spambots.
